PROBLEM STATEMENT

Recuperative Care and Short-term Post Hospitalization are a critical phase in the recovery process after hospitalization.                 


The current healthcare system often leaves patients without adequate support during the recuperative phase of their care.                              

        

After being discharged from the hospital, patients may require ongoing medical attention, therapy, and care coordination to fully recover.


Many lack access to suitable housing that leading to:                               


  • Unnecessary/high risk of hospital re-admissions due to inadequate post-discharge care.


  • Prolonged recovery times and hospital stays.


  • Increased healthcare costs.


  • Reduced patient satisfaction and overall well-being.

  

SOLUTION

We address the problem by providing specialized housing for    Recuperative Care and Short-term Post Hospitalization.    


Our facilities are designed to meet the needs of patients:                


  • Each Bedroom will have its own Bathroom for safety and privacy providing a safe space.


  • Comfortable, accessible, privacy and safe living environments, tailored to individual needs.


  • On-site medical support - in coordination with Healthcare Provider.


  • Care coordination, navigation, transitions - in coordination with Healthcare Provider.


  • Nutritional support and meal services - in coordination with Healthcare Provider.


  • Social activities and community engagement - in coordination with Healthcare Provider.

SUPPORTIVE SERVICES

Each Bedroom will have its own Bathroom for safety and privacy

providing a safe space for the patient.                                                 


While our primary service is focus on housing, we can provide the below services in coordination with Healthcare Providers:       


  • Partnership - partner with Healthcare Providers for services like IV Medication.


  • Clothing - we will provide clothes to residents in need.


  • Extended Stay – create a plan for long-term stability and aftercare support.


  • Therapy - we will work with Mental Health services.


  • IV Medication - supported - Healthcare Provider must arrange for home Healthcare.


  • IV Drug User - we don’t provide support – this is provided by specialized providers.


  • Paraplegics/Wheelchair - supported - must independently complete daily activities.

COST SAVING

  • Individuals experiencing homelessness use hospitals at a    much higher rate than people with homes. 


  • The average length of a hospital stay is 4.6 days, but the average individual without a home stay twice as long.


  • Without a place to recover, many end up back in the emergency room within 30 days.

 

  • Recuperative care can help offset the financial impact to hospitals and reduce hospital re-admissions by as much as half.

 

  • Given the average cost of $3,300 per day for a hospital stay, compared to the average $250 per night for Recuperative Care/Short-term Post Hospitalization, the savings quickly add up.

EFFECTIVENESS

  • Recuperative Care/Short-term Post Hospitalization helps break the cycle of repeated hospitalizations of individuals experiencing homelessness by allowing them to properly heal after a hospital stay.


  • Access to Housing and other Supportive Services necessary to remain healthy is a key to addressing Medical Respite.

BENEFITS

  • Approximately 75% of clients entering Medical Respite are not listed in government social services’ databases.

 

  • While in Medical Respite, clients’ information is entered into the county’s Coordinated Entry System (CES), which can help them better access Housing Resources and Social Services, including Mental Health, Primary Care, Rehabilitation and other Community Resources.

AMENITIES

  • Security/Safety - video camara to monitor facility.


  • Housing Keeping - house cleaning services.


  • Laundry Services – laundry facility available on first come – first serve.


  • Entertainment - cable television.


  • Internet - shared WiFi.

Occupancy

 

  • Residency - only residents are allowed to stay/live at the facility.


  • Healthcare Provider - an exception may be granted for overnight stay.


  • Caretaker – caretakers are Not allowed to stay/live at the facility.


  • Overnight Stay - there will be No overnight stay for Non-Residents.
