The current healthcare system often leaves patients without adequate support during the recuperative phase of their care.

After being discharged from the hospital, patients may require ongoing medical attention, therapy, and care coordination to fully recover.

However, many lack access to suitable housing that meets their unique needs, leading to:

   * Unnecessary/high risk of hospital re-admissions due to inadequate post-discharge care.

   * Prolonged recovery times and hospital stays.

   * Increased healthcare costs.

   * Reduced patient satisfaction and overall well-being.

Solution

We address the problem by providing specialized housing for Recuperative Care and Short-term Post Hospitalization.

Our facilities are designed to meet the unique needs of patients who require ongoing medical attention, offering:

   * Each Bedroom will have its own Bathroom for safety and privacy - providing a safe space for the patient.

   * Comfortable, accessible, privacy and safe living environments, tailored to individual patient needs.

   * On-site medical support and therapy services - in coordination with Healthcare Provider.

   * Care coordination and navigation to ensure transitions and continuity of care - in coordination with Healthcare Provider.

   * Nutritional support and meal services - in coordination with Healthcare Provider.

   * Social activities and community engagement - in coordination with Healthcare Provider.
